Effective treatment of chronic wounds may lower the costs, prevent readmissions, improve quality of life, and reduce the overall length of stay in a skilled nursing facility. Unfortunately, it can be difficult to get this kind of specialized care on-site. Summit Medical Consultants will be bringing experienced, knowledgeable Wound Care clinicians on-site to better manage our complex wound patients and improve their overall outcome. Working in tandem with our primary care team, our Wound Care clinicians are an integral part of care for so many patients with multiple chronic conditions, providing expertise and guiding the management of wounds to promote healing.
